Welcome to the 2012-2013 Influenza Campaign printed materials page.
| Directions: There are two different categories of information you can order: Influenza Materials and
Pneumonia Materials. Click on the links below to open an order form. At the top of each order form is
a picture of the different materials available. Clicking on the picture will open a larger view. When
you are ready to order, enter the quantity and the contact information in the appropriate spaces. To
move from blank to blank, use your TAB key, NOT THE ENTER KEY. Pressing the ENTER key will submit the
form to the Influenza Program staff for processing.
